Before you start

You generally need to be enrolled on the iBudget waiver, live in your own or family home, and work with a CDC+ consultant. You need a representative if you are not able or willing to act as self-representative.

High-level enrollment sequence

  1. Talk with your consultant about whether CDC+ fits your goals and capacity as an employer.
  2. Complete training for the consumer and/or representative (see APD CDC+ training materials and calendar).
  3. Pass readiness review activities with your consultant.
  4. Enrollment packet (forms change — use the live APD library) typically includes items such as:
- IRS Form 8821 and 2678 (household employer / tax agent related) - Program Consent - Informed Consent for Fiscal Employer Agent - Representative Agreement when a representative is used - Direct deposit and other enrollment documents listed on APD’s Steps to Enroll materials
  1. Budget authorization — the State Office issues budget confirmation (often via a Budget Authorization Form / BAF with consumer ID and approved budget).
  2. Draft Purchasing Plan with consultant assistance; submit for review; complete corrections.
  3. New-hire packets for any workers you plan to use at start, including Level 2 screening documentation.
  4. Submit plan and packets to the consultant on the program calendar (historically by the 5th of the month before the desired start month — confirm the current deadline).
  5. Begin services only after the F/EA has processed packets and issued provider IDs.

What this app does not do

CDC+ Connect does not submit enrollment packets, authorize budgets, or run the F/EA. Use official APD channels and your consultant.
